In pursuance of the requirements of the Law of Ukraine “On the Protection of Personal Data” by filling out a Questionnaire on this website to consider the possibility of further cooperation, I agree to the processing of my personal data specified in this application by the following persons: “TOV Kinderly” (LLC) (EDRPOU code – 42226467), “TOV IGR Medical Centre” LLC (EDRPOU code – 38441428), PE “Mother and Child” Medical Centre” (EDRPOU code -32371303) to ensure the implementation of civil relations, provision/receipt and settlement of payments for purchased goods/services. I have no objection to the verification by the persons listed in this agreement of the information in any medical centres of Ukraine to find out information regarding the fulfilment of my obligations under the contracts, if any, thereby realizing that the amount of information being verified may include information, concerning the state of health, and the clarification of which will be complete and sufficient to understand my social and property status, ability to pay and bear possible further property liability, if necessary. I also agree that the owner has the right to provide access and transfer my personal data to third parties without any additional notifications, without changing the purpose of their processing. Including, to check the information specified in this Questionnaire and do not deny the transfer for the possible necessary clarification of this information to third parties, by which I mean: financial institutions, mobile and postal operators, other individuals and legal entities.
